Matt, Peter hit it right on the nose. The most common cause of failure is improper handling of the bulb. Make sure that if you accidentally touch the bulb take care to wipe the entire bulb with a clean rag. The natural oils from your hands or any other source of dirt (oils, grease, etc..) combined with the intense heat generated by the bulb is a perfect recipe for premature failure.
