Some of those things are notably dangerous. I don't think most of them are an issue if they are short lived e.g. smoking does not take much attention most of the time. Studies have shown that being engaged in a phone conversation is much more distracting than talking with someone in a car. I'll be the first to admit that it's driver specific.
