The fan gets un-switched power and the controller is in the ground connection.. If the controller fails to a short circuit, you get what you are experiencing. The controller can also fail to an open circuit as well.

Did it work for a while before it went bad? I think the controller needs to be mounted to be in contact with a heat sink or air flow. There might be an opportunity for error and overheating; but I really do not know for sure. We need others to address that.
