I'd offer from recent experience that balance shaft chain, guides and tensioner do need replacement.
My 2001 LPT has 260,000 miles, and I just had it resealed. The technician said the guides were worn through in one area, and as he removed them for replacement that they crumbled in his hand.
I had all chains tensioners and guides replaced. The result is a very quiet motor. You can barely hear it running now.
