The bolts were not round, sort of 3 lobed. These shake loose. Making them tighter is not the way to go. Use some loctite blue and tighten firm, not tight.
As the engine warms, the DIC warms and expands at a different rate, so there will always be some movement combined with vibration that makes these fasteners a problem.
A bolt from the hardware store with fully round tread profiles might work better too. You have a sample.
