I caught one tensioner where the pulley was fine, but was starting to wear a groove in the timing cover because the pivot on the tensioner was worn. Every 120K is my normal interval for replacing the tensioner. The belt every 60K and the pulleys inspected every 30K. I have found pulley life to be variable, but replacement after 120K if they have not been changed before that.
