Yes, you can replace the pistons with the engine in the car, head off, pan off. Be careful of the cooling jets underneath the pistons and note that it may have split connecting rods so they only go one way. I used a set of used pistons, just be sure to clean the top of the pistons and find out the correct size, I had size AB and I installed with new rings. I had 250k for miles and still had the cross-hatching visible and no ring at the top of the piston. Be careful removing the head so you don't crack any of the timing chain guides, they are plastic and brittle at that mileage. Use two people and know that the front guide doesn't move, on the back one pivots.
