Grinding 3rd gear

1990 SPG with some 250k. I sold it to my sister a few months ago and she called in tears yesterday. Before she got it, we were using it to go up and down the steep mountains in Utah for skiing (anyone ever been up the killer hill to Powder Mtn?). By the end of the season, I discovered my tranny oil very low and filled it up, but not before 3rd gear started to whine noticably when engine breaking or going downhill with it in. Only 3rd gear. Anyway, sold the car, gave her a deal, told her to baby 3rd, etc. Now, apparently all of a sudden when she ACCELLERATES in 3rd, there is a terible grinding sound. No other problems. Even reverse is incredibly solid.

What could it be?

She is within walking distance of Mile High Saab in Denver. I told her to splash some water on her face and look like she has been crying, then go in with a plate of fresh home-made cookies and ask for help. Works for me every time.
