I think the bellows are meant to protect your rack from ingesting roadway grit and contaminants into its internals. If you don't replace them, your rate of fluid loss will increase and eventually the rack will fail due to the worn seals.
Replacing the bellows themselves isn't a terribly dirty or difficult job. You just remove the wheel rims, loosen and extract your tie-rod ends from the knuckles, then remove the tie-rod ends from the tie-rods (carefully counting the number of turns it takes to get them off). Remove the remnants of the failed boots and slide new ones into place and secure with screw clamps or nylon ties. Then re-assemble.
Not much crawling around on your back or reaching deep into the grease pit. Most of the work is done at either side, kind of like a brake job.
I'm usually a little worried if I know any of my boots up front (CV, ball joints, steering rack) are broken or failing.
If your steering is working great, spend an hour replacing the rack boots. You can forget about steering issues for a few more years or decades.
