On my '89 the shift knob comes in two pieces: the whole knob, which has a plastic keyway on the right side (sitting in car) that fits into the brass end of the shift stick itself (I don't think that it could be inverted) and a top cap (a small plastic piece about the size of a half dollar)that can come off and be put on backwards. This happened to me on a sticky day with greasy palms from an attempt to eat a hamburger while on the road and I noticed that it made no difference if it is placed top up or down. Maybe with some sticky gum you could get a good enough grip in it that it will come out as I don't believe that it is glued but rather just a press fit that may loosen.
