Actually, when you read the literature, research the topic, ( I did ) you will see that some engines are balanced internally, some are NOT and some are done w/ BOTH and I read of one engine that was balanced internally, one part, ( the front, ) and the rest by the rear, externally ! It is not just based on modern VS old but whatever the specs/ design the engine and /or engineers dictate. As well nowadays, dual mass flywheels have an important role in dampening, absorbing vibrations so could be said to be an important part of the engine balancing - or lack thereof! Some aircraft engines have a specified RPM or range that must be avoided due to resonant frequencies causing problems
