And if people would just drive safely enough we wouldn't even need seatbelts! I'm a big advocate of putting a big metal spike in the center of the steering wheel in place of the airbag instead of all other safety features. Just think how safely people would drive then, and we would avoid all the cost of today's expensive safety features. As a bonus the spike would never explode shrapnel in people's faces!
It's inaccurate to say airbags were developed for un-belted passengers. Yes, in the very early days this was somewhat true, and designs were intended to take into account the un-belted passenger, but modern airbags have always been intended as a supplemental restrain system. Since the development of multi-stage airbags serious injuries from them (other than the Takata issue) are very rare, and this compensates for the 20mph/200mph issue you mention.
As for whether the fault rate on any product is going to show up when millions examples are in use, that's not really the case with Takata. They used a propellant no one else was using solely because it was cheaper, which actually worked out fine until they further cheaped out by not using a dehumidifying agent in the problem airbags. They cut corners and it came back to haunt them.
