I grew up in Sweden with State mandatory inspections once a year testing emission, brakes (even applied brake force between left/right/front/rear including parking brake), lights, rust, bushings, joints, tires and more.
Since it was State run it was no monetary incentives for the inspectors to "sell" you on work and back in those days, people were by default fairly honest so you just took the report back to your mechanic and he fixed the problems and you went back to have it reinspected.
I think it at least kept the worst cars off the street but certainly not a fool proof system for everything.
The business climate in this country is so much harsher with everyone chasing the mighty dollar to the last drop that ripoffs seems to be part of the deal.
Here in CA you really don't need an inspection because many shops are so gung ho on making money that they inspect and sell everything if they have a hungry mechanic and a good sales person up front.
And with the lawsuit happy climate you can always push a sale since no one wants to be sued for causing an accident.
And if you didn't push the repair, they can come back and sue you anyway because you didn't.
