A couple of thoughts. The gate not staying up is probably worn out gas struts. They are not expensive and are pretty easy to replace.
Does it idle just as rough in neutral or is it worse when you're stopped but in gear (assuming it's an automatic)? You may have worn out motor mounts.
Bad milage could be bad plugs, or a bad CPS. I'm not sure that sludge would hurt your mileage while the car continues to run. Sludge would eventually kill your motor.
