So sorry. When I did the second side I made sure I kept a box wrench on the top nut for awhile until it tightened...then it gets pulled through with no wrench on it. If it's stuck you could try putting a pair of pliers on the rod and loosen the bottom nut to take the big cup off. Then you should be able to push the threaded rod up (ok you may have to bang it up to expose the top nut.
The other way I was going to get the bushing sleeve out was with my dremel...the cutting drill bit was making some headway into the sleeve before I decided to use use the rose bush tool. The original rod has short threads on one end so its suppose to lock and pull the washer through.
If its really stuck and the rod seems shot maybe beat on the cup with a hammer to loosen it.
I apologize for the optimistic instructions..The tool did take my second bushing out middle bushing cylinder and all in one quick shot.
