engine rattle after rebuild

Having done this a few times without issue, when I needed to remove the head on a 2000 wagon due to some broken exhaust studs, I opted to replace all the goods under the timing cover. This included new chains (timing and balance shaft chains), guides, sprockets and tensioners. There is now a terrible rattle from that side of the engine. Timing is true and a check of the timing chain shows that it is tight. All I can think of is that the balance chain tensioner isn't working right. Any thoughts on what I might have missed? In my mind I'm thinking the tensioner to the block seal isn't a good one causing the tensioner to have minimal oil pressure.
I'm not looking forward to doing this job again but absolutely don't want to do it without some thought of what could be wrong.
