My experience with failing TB suggests they exhibit different symptoms - the likely cause I would speculate is where the wear occurs on the TB pots . On my 1999 I procrastinated replacing the TB for several months (like I'm doing with the DIC on my 2004) resetting it when it went into limp home mode but it was never difficult to start. On the 2004 the demise of the TB started with a rougher idle, then like you while hard acceleration at speed a cel was triggered. Try doing that again and see if you get another CEL.
But it could well be that if an input from the TB is needed for starting and it's incorrect you might get rougher starts - when I was in limp home mode on the 2004 mine had trouble holding idle possibly because the cable allowed the plate too close too much,( adjustment issue) so if you are in limp hold mode try a little gas pedal when starting to test that out.
