Sounds like the belt is the least of the issues here.. First you need to track down the coolant issue, and deal with that.. 3/4 gallon is significant.. Check all of the hoses.. as well as the bypass valve on the firewall for leaking/damage.. Hopefull its as simple as an older hose bursting/leaking.
adding a quart of oil with every tank of gas? that's pretty significant. Unless you see it smoke billowing out of the exhaust under acceleration. you likely have a leak or two going on, and it may be off on the exhaust.. so you don't notice it making its way to your garage floor. I'd check the areas around the camshaft/oil pump, oil sending switch, and timing tensioner as those are the most popular candidates..
don't just throw another belt on it, unless the belt actually broke. A belt wouldn't typically just slip off. Likely the belt tensioner froze up as others mentioned. which is an easy fix.. and check all of the pulleys and components using the belt for free movement..
