Most any amplifier will have line-level (speaker) or low-level (pre-amp) inputs, or both. You need to find out what the external amp you have supports. More than likely it'll have RCA inputs for low-level, or screw-down terminals for line-level. Line level inputs are almost always VERY clearly labelled - DO NOT confuse inputs with outputs, or you'll blow something up.
If it's low-level input, you need to break out the wires from the head unit to the rear amp, split them, terminate one set in RCA and get the other set back to the rear amp. I remember reading somewhere that the pre-amp output from the factory head unit to the rear speakers is not standard, so this configuration may not work. I'm not positive though.
If it's the line-level input, you can tap into the rear speaker wires at the speaker. Just remember, any splitting like this will reduce the power and quality of sound getting to your rear speakers and might over drive the factory rear amp.
I'm not really familiar with Bazooka amps, but I'm pretty sure they come in many flavors of internally and externally amplified. If yours is externally amplified (it has no built-in amp) you need to power it with some other amplifier. It most likely would have screw-down or spring-loaded clips for speaker wire (line-level input) - run that back to your external amp. Keep in mind that Bazooka amps are not exactly designed for the high-fidelity crowd - there's a good chance it was designed to REPLACE your rear speakers. Be sure that whatever your amp puts out will not damage or destroy the sub.
Based on what you've said, my guess is that what you're trying to do is somewhere between difficult and impossible.
