They usually crack on the sides of the socket allowing them to open up and rotate off the lever. If this is the case and you still have the knob and clip, you can easily repair by cutting about a 5-7mm long ring off the open end of a black 'Sharpie' magic marker, slipping it over the socket and glueing it with cyano crazy glue. It is a tight fit and the knob will be hard to slip back onto the lever but it will work and look as new. Been using this repair on a daily driver now for well over a year...
