Alternator/Charging question

I have a few questions here. I've been helping a friend diagnose her 1992 900 convertible. On a fully charged battery and driving locally for a few days the car shut down under way. Turn the key and nothing. After the car was towed home the battery was charged again and the car started with no issue. I checked the voltage output at the battery while running and it was about 12V. It moved around a little. I was hoping to see 14-14.5v. Is this want you would expect?
The Second question is this. Does the alternator housing need to be grounded outside of it mounting hardware? If I ground the housing I'm getting 13v output while running. Still under what I would expect. There is no alt light on the dash. I would have thought that would have turned on prior to the car shutting down. It could be a bad bulb. I haven't gotten that far.
She clams the alternator was new less that 200 miles ago. I'm thinking she has a bad voltage regulator and possibly a bad dash light.
