Depends.
First, you should clean the painted surface very well. In fact, you want to rough it up with some fine sandpaper. If there is any wax on the paint, especially silicone based wax, the epoxy won't stick. You also want to remove the outer surface of paint, because it's probably oxidized and will just peel off.
If you have rust, the paint might have trouble adhering.

This is all why they recommend bare metal. Bonding to paint is iffy because it may have wax residue, or there may be an incompatibility between the paint and the expoxy. And then the bond is only as good as the bond of the paint to the metal, and internal to the paint (oxidized or flaking). And you don't want a perfectly smooth surface - you want to rough it up with some fine (400 grit is OK) sandpaper. Epoxy bonds to the microcracks in the metal surface. Also, clean the area well with alcohol and let it dry well.
