Hi David,
1. You should be able to lock the door from the outside. In fact, that's the ONLY way to lock it. Something's wrong. Never had to deal with that one so can't help you. Time to take the door panel off and see what's going on.
2. Yes, they are threaded on both ends (male fittings). Thread the end into the caliper first, spinning the hose, then tighten it. The other end goes to a little bracket thing on the inner fender. It mates to a female fitting on the hard brake line that spins.
I think that's the best way to do it. Although, I have changed a caliper or two in the past and not done it that way. I loosened the caliper end of the hose a wrench turn or so, removed the caliper, and then spun the caliper off the hose. Same way to install it.
