My tub was not nearly as far gone as yours. On mine (ceramic) the enamel had worn through making dirt and soap scum stick and collect easily. The glazer first cleaned the tub thoroughly with a really strong smelling solvent, then taped off the tub, the drain and anything we didn't want glazed. He then took a spray gun and sprayed the new glaze onto the surface. It seems I had to wait 24-48 hours before using the tub/shower.
In your case you may have to pay a bit more for the prep, but I would think the glaze process would be similar.
