Dog hits car - owners fault?

I know this sounds bizarre but a large dog (100lb) escaped from it's owner and proceeded to run head first into the passenger side door of my car creating a large dent. The dog seems to be fine but and I hope the owner brought it to the vet for an x-ray. My car: the estimate to repair the very large dent is $1500. (believe me, I was shocked)
Details: I was driving by a park where lots of folks bring their dogs for walks. Speed was quite slow as this is a neighborhood street. All of the sudden I see a dog bolt across the park, across the sidewalk, and nail my car with a large THUMP. I pull over to make sure the dog is okay. The owner comes to retrieve the dog and I notice the damage on the car. She admits the dog (on a leash) has run away from her. I say "your dog hit my car and it looks like it damaged the door" but she says "I thought my dog missed your car." (there's no way you can admit to that after hearing the loud THUMP). Poor dog, but he seems okay. She continues to say "there are no witnesses" I tell her, "I'd rather not call the cops and get insurance involved... so this is all on your own conscience to do the right thing" As soon as I say that, she shuts up and we exchange information.
I was hoping this could be settled in a civilized manner but it looks like people don't really want to be responsible. Today, I made a police report and called my insurance company. So who is at fault here? Is anybody at fault? Or just a freak accident? Is this an auto insurance issue or dog owner's homeower's insurance claim?
If I was the owner of that dog, I would be seriously concerned about the health of the dog and would have taken the dog to the vet. Are these records subpoenaable if it goes to court?
