The brain does a lot of things automatically when you are talking with someone, including evaluating body language and gauging the distance of participants to determine speaking volume. The former is a big part of the reason why people get tunnel vision while talking on a cell phone, the latter is a big part of the reason people speak at high volumes on the phone. It's unclear from everything I have read on whether this behavior can be overcome by training or whether it simply varies from brain to brain. But I think it's fair to say the average individual is improperly prepared to carry on any sort of conversation - especially one by cell phone - while operating a car.
Text messaging and other technology is a whole different world. I know plenty of people who can barely handle using a mouse and keyboard at the same time (like playing video games, or using Excel) - it's hard to believe such a person can operate any electronic device and drive at the same time. I am plenty good at multitasking, but the different interfaces and focal lengths involved in text messaging and driving, for example, put a strain on me I so can't do either elegantly. So I don't try.
