The Ford Taunus V4 engine that Saab used in the 95, 96 and 97 (Sonett) from 1967 through 1974 (in it's USA cars) has a balance shaft. In that engine the crankshaft stops short of the timing cover and the balance shaft projects through and has a pulley to drive the belt that powers the alternator, water pump and fan. The balance shaft turns anti-clockwise (viewed from in front) so a special fan with reversed blades was used. The balance shaft just had a shell bearing (like a cam bearing) at each end so over-tightening the belt would eventually lead to wrecking those bearings. Otherwise a very beefy little industrial engine.
