I had a similar problem on a 91 9000. It turned out to be the check valve on the hose leading to the headlight washers. It broke, so that the fluid just slowly seeped out of the headlight washer jet. If you can't get to the bottom of your tank (sorry I don't know how) see if you can trace the hose from the headlight washer back.
