Seems that the automatic adjustment does not work. The cables must have
proper freeplay and they must run freely. Maybe the mechanism inside brake
caliber is corroded (have you changed brake fluid regularly). The lever in
the caliber must come all the way back when brake is released, if not the
automatic adjustment don't work. The hexagon screw is only for maintenance,
to reset the mechanism when you change pads.
