In the "old days" the 45K service was just an oil change and a look over of the car.
The 30, 60 and 90K services are the big ones.
Since many of the services are free up to a certain mileage, what happens is that the car manufacturer tries to hold off with as much as possible until the car is over that free mileage interval and then they come back and slam you with a hefty cost to catch up with anything that was skimped on earlier.
Your car should have had a major service overhaul at 30K miles and then it should not need much more than oil changes and a full look over until 60K miles.
Do you have any paper work what was done at 30K?
Did they specify what was going to be done for the 45K service?
I have some customers driving from San Diego to my shop. It's kind of stretch but they don't mind.
