So for all of you who have been waiting for the next chapter with eager anticipation: Yesterday I cleaned the MAF sensor (with MAF sensor cleaner), the MAP sensor (with carburetor cleaner), removed and cleaned the throttle body (with carburetor cleaner), and replaced the throttle body gasket (after thoroughly cleaning the mating surfaces between the manifold and the throttle body).
One of these, or a combination of these, solved the problem. I suspect it was the throttle body or gasket - the sensors seemed pretty clean to start with. The idle is now EXTREMLY stable. No idea why the problem seemed more pronounced in reverse, but it's a happy camper once again, and will hit the 200K miles mark today! (I got it a cake to celebrate.)
