I agree, it's a combination of issues and the PCV will be the boogeyman. Easy target. It does deserve much credit, but certainly not all -- maybe more than you think. It seems to be the enabler of a bunch of bad things. Heads have been loose since 97 without the T5 engines going bad.
Clogged small circuit was one failure mode, but the valve itself fails open, and you get some boost pressure back to the VC. Maybe not tons, but more than evisioned trying to get out the large circuit. Also, the large circuit is restricted (3mm/6.5mm/10mm+check valve depending on which mod you have). The hoses are big but the closer to stock you go, the less the system handles. Mod3 was the only one to get close to what a T5 could handle under boost.
New fuels is also an excellent point; Saab is not the first manufacturer to extend late model engine warranties due to sludge. And fuels seem to change today without much concern for side-effects (MTBE, while a different issue, is an example.) It would be interesting to see what the technical deal was in the case of Toyota and VW/Audi. BITOG have any comment on fuels?
