I replace the tensioner at 120k. so if you are close to that yea its a good idea to replace it. And of course, regardless of mileage, you can visually inspect the tensioner ... does it track straight? does it bounce excessively? Maybe your mechanic sees evidence that it is worn, and in that case its good to change it out even if its much younger than 120k.
FWIW I change out the pulleys at 60k, so consider doing those as well depending on your mileage. The pulleys are plastic and tend to self destruct. (esp the smaller one)
I cant say with authority, but an extra $200 to do the tensioner seems high to me.
