Could be a sticking purge valve. If the purge valve sticks during the test, then the vacuum that's suppost to go to the tank never reaches the tank. So the system thinks there is a large leak.
I have also seen small cracks on the top of the tanks where the plastic fittings for lines or the pressure sensor are mounted. The plastic fittings will crack were they are welded to the plastic tank. These cracks can cause intermittent faults depending on temp, and fuel level. You usually need a smoke machine to find these leaks.
