Went back to the car after lunch break and a clear head. On #3, it was cross-threading. Used a prybar to align the mount while inserting rear bolt. All is well and torques down cleanly as expected so I think I'm OK on #1 too.
Still unsure about question #2 and the one "extra" hole. My old mount definitely did not have a 4th bolt as it shows no wear signs. Car appears to have some threading as I look closely. Maybe the design changed over time?
