Or rather, mileage. Doing a simple drain and fill at 30K drops the average age of the fluid to 15K. Continuing the procedure every 15 K thereafter keeps the average to between 15K and 30K through the life of the vehicle. Maybe first change at 40K and every change 20K after is more nearly optimum from a cost/benefit standpoint, but too often is always better than not often enough when one considers cost of maintenance vs. cost of repair.
Myself, I think (my records currently are off with Mrs. G and the wagon) I did 50K/25K, until I started a regimen of 2K mi of heavy towing per year - then I changed every year (about 12K). Now that's in the past, so I'll drop back. I have part of one change worth of fluid left in the garage, and will probably change every other year (this is the year).
I think the anecdotal "problems between 30K and 40K" are very rare and related either to an original defect in the gearbox or to extremely severe conditions of use. This is, after all, a Toyota transmission and one should expect 60K worth of reliable service no matter what with zero service.
As an alternative to frequent drain and fill, one could do a complete flush every 50K or 60K, but the amount of fluid required is quite high as compared to a simple drain and fill, which replaces about half.
