The single hose going into the CPV should be hooked directly to the heater core. They the hose that goes back to the engine from the CPV should be connected to the other end of the heater core. Remove the CPV, find a brass fitting at the hardware store to put the in the 3rd hose (with a nice hose clamp), zip tie the loose hose to stabilize it and you should be fine. Keep the electrical connection to valve intact and plug the vac. line.
