First of all I agree 100% that you should replace the tensioner. Mark's suggestion to do it as a maintenance item is right on. I do mine every 120k, Mark's suggestion for 60K might be more wise.
about the tensioner:
- As the spring in the tensioner wears its vibrates and bounces more. Eventually the serp belt will pop off and or get shredded.
- As the tensioner pulley bearing fails the pulley will wobble and begin to chew into the timing cover. The wobbling will cause the serp belt to pop off and or get shredded.
*but* I don't believe that the tensioner could *cause* engine vibration.
you say it smooths out at higher RPMs ... at idle how high do you have to go to ? 1000? 1500? Its not uncommon for folks to bump up the idle speed (with a Tech II) to smooth it out a bit at stoplights.
