Funny how such a minor piece of equipment can make a car useless once it rains. I think you'll find that this problem is ridiculously expensive to have someone else fix, and only minorly inconvenient to do with the help of a friend.

Pull the wiper out of the hood.
The spike on the end of the wiper fits into grooves in the pulley. After 15 years, these two pieces would only fit together properly when certain grooves were lined up together. Find the position in which they used to fit and it should slide in without too much effort.

The way the wipers work is that the motor moves a rail back and forth. The pulley is fixed to the rail by means of a 1/16 twisted steel wire.
(The wire is attached to the end of the rail with a stopper, runs along the rail, under the pulley, around the pulley, and back under the pulley to run along the rail again. The other end of the wire is fixed by a stopper to a spot further down on the rail.)
When the rail moves toward the driver's side, the pulley turns counter-clockwise and vice-versa. This only works well because there is a notch in the pulley big enough to hold a third stopper on the wire. It's this notch that allows the wire to effectively grab onto the pulley.

I had the same problem as you did, and found the following solution:

Once the wire has slipped off the wheel, I don't think it's going back. You'll need about 5 feet of 1/16 twisted steel cable, a permanent marker, some stoppers, pliers, and a pair of wire cutters from a hardware store. Clip the original cable off the rail, making sure to cut at the end. The original length will help you figure out where to place the stopper on the new cable.

1. Take a few feet of cable and attach a stopper to one end. (Squeeze it tight with the pliers.)
2. Match the new cable to the old and mark a spot where the next stopper should be placed.
3. Thread the open end of the cable through the end of the rail and pull it inside until the stopper hits the outside of the rail.
4. Place a stopper on the cable at the marked spot, and lay the cable down on the inside of the rail.
5. Place the pulley on the rail, notch down, so that the stopper fits into the notch.
6. Roll the pulley towards the drivers-side, so that the wire begins to wrap around the pulley. Wrap the wire all the way around the pulley, so that it comes out from under the pulley facing the passenger side and threads through the hole in the bottom of the rail.
7. With the pulley and rail in your right hand, and a friend holding the wire taut, grab the wiper with your left hand.
8. Move the rail and pulley to fit the pulley under the metal tube where the wiper fits.
9. With the wiper facing a position of rest, place it through the tube and into the pulley.
10.Have your friend thread lots of stoppers onto the end of the wire (gives you more room to clamp), pull tight and clamp the end stopper down hard. That's it.
