Doing a careful trans oil change, surgical conditions, with mabe an extra flushing of oil, would you add 1/2 teaspoon of fine, hardened metal chips with the last quart?

Don't have to, because Saab does it for you, by was of a self destructing reverse gear. This is due to 3 conditions:

1 - reverse has no syncro (not unusual)
2 - clutch often does not fully disengage the disc, due to some drag
3 - must have car in reverse to pull key

Almost every box torn down has chewed up reverse gears. Check Jeff C's site for a pic. Reverse gears are adjacent to the pinion bearing ... my bearings showed fine divots due to metal particals.

This problem can be minimized by habbit ... avoid shifting to reverse with engine on when initially starting, or parking after use:

1 - If you need to back up with 1st start, leave it in reverse from cranking to back-up (no shift), as much as possible.

2 - If you move forward to park and exit, turn off engine BEFORE putting in reverse ... with eng off, may need to shift into another gear temporarily to nudge things before reverse can be selected.

3 - If you must engage reverse with engine on:

a) make sure you are stopped
b) be patient .. don't force it
c) select another syncro'd gear ... like 3rd or 4th, before grabbing reverse. This stops internal motion in the box due to clutch drag.

This problem is made worse, in all cases, when you are stopped on an incline, which makes the clutch drag issue noted more intense.
