Eric's second possibility is a very likely one I'd say, as it's just what happened to me. The indirect piston, which pushes on the yoke, get farther and farther out of the caliper as the outer pad wears. The exposed portion of the ground piston surface corrodes, forming tight eruptions of rust so that it will no longer fit into the bore in the caliper -- i.e., it won't go back in. If you could force it back in, the rough rusted portion would tear into the seal (worst) or fail to seal because of its roughness. Either way, the brake will leak fluid and soon fail.
