first is the spring. If it gets tired or breaks inside the lifter. Second is when the inside bore becomes scuffed/coarse/worn (from a number of causes) and the cap inside siezes in any position. One way, it will cause a lot of very loud tapping and a loss of performance (from the cap siezed in a compressed position). Sometimes this is a temporary condition when the lifter is compressed at engine shutdown and takes a few minutes after crank to quiet down.
The other way (cap seized in the up position) may cause the valve to contact the piston (this is normally what is referred to as "Catastrophic"). I have even seen where the hydraulic lifter failed in the up position and pushed a rocker stud right out of the head. Not possible with overhead cams though.
One thing to remember is to put lifters back into the same hole they came out of. And really, they are not that expensive if you just want to replace them. Good luck
