Well, I knew there was a problem with the fuel system because of low gas mileage, even after a tune-up. Turns out the previous owner compensated for bad running by increasing the fuel mix setting, causing it to run rich at all times. After I readjusted the fuel mix setting, the main problem was difficulty idling, caused by all the various vac leaks. In general, a thorough cleanup of the entire engine and components, wiring, connections, replacement of leaky units solved most of those problems. Replacing the questionable fuel injectors fixed the remaining issues.
As far as the vac leak causing lean condition - initially, yes, which is then compensated by the O2 sensor. A wildly fluctuating idle is often a symptom of the O2 sensor overcompensating between lean and rich. Perhaps if the leak is great the lamba system locks into the rich setting, which when coupled with a sizable vac leak will cause acceleration and drivability issues. Beyond that I wouldn't know how to diagnose.
Another possibility is a faulty ignition system where the spark is cut or minimized so that it won't ignite the fuel under select conditions. Excess unburnt fuel shows itself as black smoke in the exhaust.
