Since your idle smooths out when the fans come on, another possibility is the voltage regulator/brushes are on their way out. When the alternator has a higher load request it loads the whole system. Some part of your system isn't getting enough current/voltage under normal ops. Might want to check and clean ground points in the engine compartment especially grounds for LH (on the head near the front (?), alternator, and grounds near the rad lower cross support.
