Bad master cylinder. My '89 SPG had very similar symptoms. It turned out that a few things went bad with the master cylinder:
- the spring which pushes the piston back to the rest position had broken into 3 pieces
- the seals were worn
- there was a ring of varnish at the entrance to the cylinder that also prevented the piston from returning to the rest position - it also lifted the lip of the rear seal a bit so it sucked in a bit of air.
I was lucky that there wasn't any pitting of the master cylinder bore, so it was easy to put new seals and a spare spring in it.
It's worked well for about a year. But ... a few months after I rebuilt the master, the slave failed - probably because the fresh fluid washed out the crud that had sealed it for years. Since I replaced that, it hasn't had any problems with the clutch.
My guess as to why it works well when it's cold and dies after the car warms up is that the viscosity of the fluid is higher when it's cold and decreases as it warms up, so it leaks back past the front seal in the master. And the seals become more pliable as they warm up.
