My experience is that usually, you don't. You end up replacing the caliper. They aren't usually too expensive. I've found the time spent trying to remove one better spent on other things.
If you go the "extractor" route, be careful, extractors are brittle, but made of very hard metal.
After replacing, use caution not to overtighten the bleeder and use some sort of antiseeze compound on the threads.
