With the comment about the bumps and popping out, I would be checking the motor mounts. Hard to see, but if you look from underneath at the driver's side, make sure both bolts are there and tight on the motor mount. I have had two cars that one of the bolts was missing, and the car would go to neutral uncommanded due to road surface irregularities.
Once you know all three motor mounts are all solid, rigging the shift coupling would be the next step. If you rig the shift coupling and the mounts are dead, gear disengagement will probably still prevail.
If all that doesn't solve the problem, I would then guess it's internal to the gearbox.
