I worked through the same issues on a 1991 vert. So, you have a check valve on the engine bay side of the firewall just in front of the driver. There is a T fitting that has a coupling that has a small and large hole. This coupling will split and cause the vacuum to leak, thereby changing the HVAC flaps when you push on the throttle and the manifold vacuum changes.
The tank in the rear is just negative pressure storage -- there is an identical coupling back there on the tank that couples to the hard plastic line -- same part number as the one on the firewall.
It is also possible that you have a bad check valve on the firewall other than the coupling failure. And, it could be that the vacuum reservoir tank in the rear has a leak too.
So, in summary, after chasing all the same off and on for weeks, it turned out to be a split in the rubber coupling against the firewall that I couldn't see.
