I'll be that, by pumping the clutch one too many times with the spacer in, you hyperextended the slave cylinder and clipped out a chunk of the outer O-ring.
Been there, done that.
After you check to see that the bellows is full of brake fluid and count to 10, you'll need to take it all apart disassemble the slave cylinder, and replace the O-rings. In my case, I had a set of O-rings on hand that I'd bought years earlier, so redoing it only cost a few hours.
The good thing is that the spacer is still in there, so you can take it all apart, order a set of slave cylinder seals from eEuro, rebuild it, and put it all back together.
This is one of those cases where having a helper who you can bribe or otherwise coerce to sit in the drivers seat for a minute or two and press the clutch pedal, makes it easier to get the spacer out. It may cost you a dinner out.
