has badly-pitted contacts. The relay is far under-rated for the amount of current that toes through it.
It's a small, cubic relay in fuse box relay position E.
When what you describe has happened to my car, I've taken out the relay, opened it up, gently took the hinged contact off, cleaned up the contacts with a 1/4" Dremel wire brush, and put it back together. Don't lose the small return spring unless you have a spare relay.
The contacts should be shiny. You can see how pitted they get here:
